Blood film comments

Some common terms used in blood film interpretation;

Acanthocytes- Irregularly-edged cells seen as an artefact or due to liver disease, alcohol excess, abetalipoproteinaemia among other inherited disorders

Anisocytosis- variable red cell size- often reactive e.g sepsis/anaemia

Anisopoikilocytosis- both of the above indicating a stress on the marrow

Atypical lymphocytes- used to describe virally-activated lymphocytes or less commonly lymphocytes seen in lymphoproliferative disease such as CLL (report will usually advise if the lymphocyte morphology and blood count are a concern for a lymphoproliferative disease)

Blasts- seen with a leukoerythroblastic film (see below) or acute leukaemia

Fragments/Schistocytes- Sign of intravascular haemolysis such as disseminated intravascular coagulation or microangiopathic haemolytic anaemia- feature of significant concern if present in large numbers, especially if associated with thrombocytopenia or deranged coagulation screen

Howell-Jolly bodies- small nuclear inclusions in red cells seen in splenectomized patients or hypo/asplenic patients including. This can include patients with coeliac disease who can have hyposplenism

Hypersegmented neutrophils- indicate B12 or folate deficiency (as do macro-ovalocytes for B12 deficiency especially)

Hypochromia- seen in iron deficiency and thalassaemia

Left shift- When white blood cells in circulation are immature, commonly seen in response to infection

Leukoerythroblastic- Immature white and red cells seen in blood; usually sepsis forcing immature cells from the marrow or infiltrative process invading the marrow

Microcytes- Small red cells seen in iron deficiency and thalassaemia

Poikilocytosis- variable red cell shape- often reactive e.g sepsis/anaemia

Polychromasia- This means red cells of different colours and thus stages of maturation (mature red and immature blue/red cells). Indicates marrow stress, or haemolysis

Reticulocytosis- raised reticulocytes (immature red cells) seen with marrow stress or haemolysis

Spherocytes- homogenous, spherical red cells seen with haemolysis or congenital hereditary spherocytosis

Target cells- seen in liver disease and inherited haemoglobin disorders

Tear-drop cells- seen in myelofibrosis

Malignant encyclopaedia

Acute leukaemia is a cancer of the bone marrow that typically comes on over days or short weeks, and progresses briskly if not treated. There are different types of acute leukaemia: broadly myeloid and lymphoid.
- Acute myeloid leukaemia (AML) is a cancer of an early myeloid cell (responsible for neutrophil, monocyte, platelet production among others) which leads to ‘filling’ of the marrow with malignant cells. Patients usually present with pancytopenia, and may have a high white cell count but this is not always present. It is most commonly seen in the elderly but can be seen in all ages (less common children). It can rarely present as a ‘tumour’ of AML – a ‘Chloroma’. AML is a challenging disease to treat- aim is to get remission with therapy, but many patients relapse and may need allogeneic transplant, or if not fit, palliative management.
- Acute lymphoblastic leukaemia (ALL) is a cancer of an early lymphoid cell (responsible for B and T lymphocyte production among others) which leads to ‘filling’ of the marrow with malignant cells. Patients usually present with pancytopenia and a high white cell count but this is not always present. It is most commonly seen in children with very good responses to chemotherapy, but can be seen in older patients where it is often hard to treat.
B cell – lymphoid white blood cell which can both present foreign antigen to other parts of immune system and also produce antibody-these cells are known as plasma cells (they are the cell which is malignant in myeloma).
Blast – Primitive white blood cell seen in bone marrow. Most common are myeloid blasts (makes neutrophils, eosinophils, basophils, monocytes and platelets) and lymphoid (produces lymph cells- B and T cells). Normal up to 5% in bone marrow. A finding of 20% or more blasts in the bone marrow is in keeping with acute leukaemia (can be myeloid (AML), or lymphoid (ALL)). Rarely seen in blood in very unwell, non-leukaemic patients but most often a sign of bone marrow disease, usually leukaemia.
Burkitt lymphoma – the most rapidly proliferative high-grade non-Hodgkin lymphoma (see lymphoma section). Can double in size in as little as 19 hours. Often associated with EBV and HIV in the developing world. High risk tumour lysis syndrome.
Chemotherapy – it is unnecessary for a non-haematologist to know chemotherapy regimens used (haematologists struggle to remember them all!) but some key points to know
- Cycle length varies between diseases but are often between 21-28 days. Some patients will have a fixed number such as 6-8. More therapies are now continuous cycles (continued until the patient progresses)
- Neutropenia tends to occur at days 8-14 of chemotherapy
- GCSF is usually given only in severe sepsis, or if a patient is particularly prone to infection (NICE guidance)
- Bleomycin is often given in Hodgkin lymphoma chemo and can cause lung toxicity. If seeing a patient who has had bleomycin and presents with progressive breathlessness without obvious acute infection symptoms, this should have urgent haematology/respiratory review
- Purine analogues such as Fludarabine and alkylating agents e.g Bendamustine, can cause transfusion related graft versus host disease so patients who have received these should receive irradiated blood products (see irradiated blood entry and “irradiated module”)
- Rituximab can be associated with acute and prolonged neutropenia
- Many myeloma chemotherapies and Vinca alkaloid chemotherapies e.g vincristine or vinblastine which are used in lymphoma and some solid organ cancers cause neuropathy which can be disabling. Any evidence of this needs a full assessment to allow chemo reduction if needed.
Chronic lymphocytic leukaemia – Cancer of lymphoid cells which predominantly circulate in blood. Often an incidental finding. Patients can often be observed for many years as if low risk, prognosis is over 18 years. Features that merit referral/treatment would be symptoms (bulky lymphadenopathy/splenomegaly, night sweats/fevers, weight loss), a rapidly rising white cell count (doubling time less than 6 months, although some patients may have a WCC of 100 or more and still not need treatment) or when there are so many CLL cells, they fill the bone marrow and patients develop cytopenias e.g anaemia or thrombocytopenia. CLL can also associated with autoimmune phenomena e.g ITP/haemolytic anaemia.
Chronic myeloid leukaemia is an overproliferation of white cells, officially a myeloproliferative disease but usually seen as a separate entity. The absolute majority are associated with the Philadelphia Chromosome which is a fusion of the BCR and ABL genes leading to overproliferation of white cells. Tends to present with very high white cell counts but of mature cells e.g neutrophils and basophils. Blasts only seen in advanced disease. There may also be splenomegaly and constitutional symptoms. Response rates to targeted therapies such as imatinib are very good, with trials ongoing to see if patients can be ‘cured’ of what was previously thought to be an incurable disease.
Diffuse large B cell lymphoma (DLBCL) – the most common high-grade non-Hodgkin lymphoma (see ‘Lymphoma’ entry), often treated with up to 6 cycles of ‘R-CHOP’ chemotherapy if patient well enough.
Essential thrombocythaemia - is an excess of platelets and managed with aspirin/hydroxycarbamide chemotherapy. Associated with thrombosis risk but once count gets very high (>1000) there is potentially a bleeding risk also as the huge number of platelets ‘use up’ von Willebrands Factor and this can lead to bleeding. Long term risk of myelofibrosis/AML transformation.
Follicular lymphoma – the most common low-grade non-Hodgkin lymphoma (see ‘Lymphoma’ entry), cannot be cured but low grade disease can be indolent for many years on watch and wait. Common therapies include an anti-CD20 with chemo e.g Rituximab+bendamustine and Obinotuzumab+CVP. Patients usually get 2 years of maintenance rituximab/obinotuzumab.
Graft versus host disease – seen after allogeneic (from donor) transplant. The donated cells are matched well to the recipient but there is always a degree of mismatch and the donated cells which become the recipient’s new immune system can see the rest of the body as ‘foreign’. This leads to immune damage, Tissues often involved are skin, liver and gut. Managed with immunosuppression.
Irradiated blood – this is needed in patients with severe immunodeficiency, particularly after T cell depleting treatments as lymphocytes in the donated blood could invade the recipient bone marrow, grow and ‘attack’ the new host with transfusion-associated GvHD which is almost universally fatal. Incidence has been reduced since blood products are now leukodepleted when donated. (see ‘Irradiated products’ module for more information).
Lymphoid – referring to lymphocytes, a type of white blood cell, e.g B or T cell. Cell/tissue from lymph gland or thymus.
Lymphoma – a cancer of lymph tissue but can present in other tissues, bone marrow or blood (uncommon)
- Lymphoma is often overcomplicated by acronyms but all the non-haematologist needs to know is there are 3 main types;
  - Hodgkin lymphoma or Hodgkin disease (HD) – usually seen in younger people, but further peak in older age. Responds well to treatment, especially in young people. Need irradiated products lifelong as Hodgkin lymphoma has complex interaction with immune system leaking to decreased white cell (including T cell) function
  - Non-Hodgkin lymphoma
    - High-grade – more rapidly growing lymphoma, has potential to be cured if patient fit enough for intensive chemo (examples – DLBCL, Burkitt). Patients can present very unwell.
    - Low-grade – slower growing but due to this cannot be cured in almost all cases. Treatment plan is usually to gain remission, and accept risk of relapse under monitoring.
Lymphoma vs Leukaemia –It can understandably be confusing to see the difference. Both are cancers of white blood cells but are different conditions. Lymphoma is a cancer of white blood cells from lymphoid tissues- usually lymph nodes/spleen but can affect any organ. It is uncommon in blood. Leukaemia is a cancer of the bone marrow and presents with pancytopenia and/or a high white blood cell count. Rarely presents as lump of tissue. They are treated differently as respond to different chemotherapies.
Lymphoproliferative disease – Sometimes used to describe diseases like Chronic lymphocytic leukaemia (lymphoma and myeloma would officially be in this category but are rarely described as such).
MGUS (monoclonal gammopathy of uncertain significance) – a paraprotein (e.g IgG kappa) which is detected in blood, often incidentally but with no end organ damage as would be associated with myeloma. All myeloma cases are thought to come from MGUS but at 20 years after detection, only around 18% will progress to myeloma. 6-12 monthly monitoring is usually recommended, observing for the protein level and monitoring for any end organ damage (anaemia, hypercalcaemia, new/progressive renal impairment, bony pain). IgM paraprotein increases risk compared to IgG/IgA but still only 50% will have progressed at 20 years.
Myelodysplastic syndrome – Disorder of the bone marrow where acquired mutations in haematopoietic cells lead to the bone marrow ‘filling’ with ineffective blood cells. Usually seen in older patients. Will usually present with one or more cytopenias. Many patients will die before it needs any treatment, common treatment for older patients is supportive transfusion, occasionally chemotherapy or stem cell transplantation if young as the long term risk is leukaemia.
Myeloproliferative disease – diseases of excess production of myeloid cells. They are associated with specific mutations in most cases and all have a risk of transformation to acute myeloid leukaemia. The diseases are; polycythaemia vera (previously known as polycythemia rubra vera), essential thrombocytosis, chronic myeloid leukaemia (although usually classified as a separate disease) and myelofibrosis.
Myelofibrosis - causes fibrotic material to fill the bone marrow, leading to expansion of other haematopoietic tissue-can cause massive hepatosplenomegaly, cytopenias and a risk of transformation to acute myeloid leukaemia.
Myeloma – cancer of plasma cells (fully mature B cells which normally make immunoglobulin/antibody). Problems arise from the cells themselves (bony pain, anaemia, hypercalcaemia from bone erosion and fractures including spinal cord compression) and the antibody they produce (renal impairment and less commonly hyperviscosity). Usually a disease of older people, cannot be cured but prognosis continues to improve.
Night sweats- Seen in lymphoid disorders such as lymphoma and CLL, but also infections and autoinflammatory disorders. Significant night sweats are described as drenching, with the patient needing to change their bedclothes and bedding.
Polycythaemia vera – a myeloproliferative disease which causes predominantly an excess of red cells but can be associated with raised white cells and platelets. Usually managed with venesection. Over 95% of patients positive for JAK2 Exon12 mutation. Long term risk of myelofibrosis and AML in some patients.
Stem cell transplant- used to consolidate chemotherapy or to increase chances of a cure in high risk diseases
- Autologous stem cell transplant – more helpfully often now called ‘High Dose Therapy with Autologous stem cell rescue’. This explains more clearly the role of an autologous transplant, where the patient has their own stem cells harvested so that more intensive chemo can be given, and then the patient’s stem cells are re-infused to allow rapid recovery from neutropenia. Used in myeloma and lymphoma.
- Allogeneic stem cell transplant – used most often in acute leukaemia but sometimes refractory lymphoma and also inherited bone marrow failure/immunodeficiency syndromes. High intensity chemotherapy is given to the patient followed by infusion of donor, HLA-matched stem cells. Only chance of a cure for relapsed AML but high risk- around 10-30% mortality from treatment alone. Graft versus host disease common complication.

T cell – lymphoid immune cell used to support/ directly kill pathogens/malignant cells.

Key facts about data control by AppYourself;

The servers used by AppYourself are located in Europe (and thus protected by GDPR) and the US.
The data is hosted on AWS infrastructure, but AWS does not store any user-related data. User-related data is stored by AppYourself in Google Firebase. The storage is located in USA.
Although the Google Firebase server (based in the US) is not secured by GDPR they operate under the EU-US privacy shield.
